I love the feature of having the reminder every 12 months or 6 months but Once you get the reminder there is no way to say its done or mark it off. They show last activity but there should be a way to mark the activity as a review and that way it can report back and schedule 6 months from there for the reminder. The only way to know its complete is the notes which is fine but if you are the contact not looking for review specifically you might not know when they call they are past due. Keep the reminder but implement a tracking system that would be interactive.
